Chelsea are the champions of England for a fifth time in 12 years after a Michy Batshuayi strike with just eight minutes remaining sealed a famous win at the Hawthorns.

It was a real rollercoaster of a tie. Chelsea failed to convert their chances and West Brom put up a good fight, but the Baggies were eventually broken down by the dominance of Cesc Fabregas and co.

On the subject of Fabregas, the Spaniard produced a rather memorable moment shortly after the game.

As Sky Sports' Geoff Shreeves approached a number of Chelsea players for an interview, Fabregas proceeded to drop the f word in front of the millions watching at home:

Shreeves hastily reminded Fabregas that he was on live television, but c'mon Geoff - it's bound to happen after winning the Premier League title.

It reminded us of that infamous Balotelli moment after winning the FA Cup final against Stoke City in 2011. At the end of the interview, Mario said that his season was "shit" after, quite frankly, a poor campaign at Manchester City.

Antonio Conte's men have dominated the Premier League for most of the season, with an incredible 13 game winning streak between October and January cementing their place at the summit, where they have been unable to be budged, despite Tottenham's best efforts in the second half of the campaign.

Chelsea's latest triumph is also John Terry's last one as club captain, with the former England skipper set to depart Stamford Bridge at the end of the season.

He leaves with five league title, five FA Cup and three League Cup winner's medals which he has won over the course of 713 appearances with the Blues.

Conte will be hoping he fares significantly better in his second season than Ancelotti did though, with the former AC Milan boss receiving his marching orders from Roman Abramovich after a trophyless 2010/11 campaign.

Congratulations Chelsea, the 2016/17 Premier League champions.
