When Chelsea's Premier League winners collect their medals and trophy on the last day of the season against Sunderland one man will be opening his post to find his medal a few days later.

It's as inevitable as it comes that Chelsea will be Premier League champions. Following Spurs loss to West Ham and the Blues win against Middlesbrough on Monday night it could happen as early as Friday.

If Antonio Conte's side beat West Brom and the Hawthorns then they'll be champions of England again, and we'll no longer be able to say that about Leicester City, but even if they don't win they'll still have home games against Watford and Sunderland to secure the trophy.

That makes Chelsea as much the champions as you can be, needing to win one of those three games or indeed three draws should be easy for them, however one of their Premier League medals will be heading to China.

After playing nine games at the beginning of the season Oscar will be due a medal as the Premier League rules state that a player with five or more appearances gets a medal.

The last time Oscar played for Chelsea was in November in a game against Tottenham and since then he's moved to the Chinese Super League for a ridiculous amount of money.

Nathaniel Chalobah, John Terry, Kurt Zouma and Ruben Loftus-Cheek are the Chelsea players with less than 10 appearances this season who will also be awarded medals for their contributions.

Second choice goalkeeper Asmir Begovic has only made one appearance in the league and so won't win a medal, the same as Nathan Ake, despite being brought back from loan in January from Bournemouth.

It will be John Terry's last league title with Chelsea.

The Blues won't lift the trophy till the last day of the season and with John Terry set to leave Stamford Bridge at the end of the season it's expected that he'll be given a fitting send off by accepting the trophy in front of an adoring home fanbase.
